INSTRUCTIONS: This form should be used to report the "Official Work Week" and "Official Work Schedule" for employees as required by the Department of Personnel and the Department of Labor. Departments must notify the Department of Personnel and affected employees at least two weeks before the change can be implemented. The requesting department must receive approval from the Department of Personnel BEFORE any permanent change (longer than 30 days) in the work schedule can be implemented.

DEFINITIONS

JOB CLASSIFICATION: Use the official job title as it appears in the current compensation ordinance, e.g. Heavy Equipment Operator I, Executive Secretary I, etc.

EMPLOYEE NAME: Enter the employee's last name, first name, and middle initial; or define the work group

OFFICIAL WORK WEEK: For most City employees, a work week, as defined by the Fair Labor Standards Act (FLSA) is a fixed and regularly recurring period of 168 hours - seven consecutive 24 hour periods. The work week does not have to coincide with the calendar week and can be different for different employees or groups of employees. Examples: 12:00 a.m. Sunday to Midnight of the following Saturday, 5:00 p.m. Thursday to 5:00 p.m. of the following Thursday, etc. For those public safety employees with an extended work week/work cycle (such as Uniformed Fire Personnel and EMS), report the approved work cycle currently utilized.

WORK DAYS: Enter the days of the week that the employee is to be scheduled to work. If the days off vary from week to week, please note this.

WORK HOURS & MEAL PERIODS: Enter the beginning and ending work times and the amount of time allotted for scheduled meal periods (if any). Be sure to note if this employee or group of employees is required to work rotating or varying shifts and cite all normal variations or conditions.

VARYING SCHEDULES: An employee's schedule may vary as a regular condition of employment. For example, a Secretary may routinely work 8-5, Monday-Friday, but once per month may be required to attend a commission meeting. Assuming that the meeting normally lasts four hours, a permissible practice would be to submit a work schedule which indicates in this column: "Yes. Employee is off on Friday 1-5 p.m. in any week(s) he/she attends a commission meeting."
